Healthcare startup Praan Health has opened its first integrated physical centre, The Fifty+ Clinic, in Bengaluru’s Koramangala neighbourhood, extending its home-based care model into a dedicated facility for people aged 50 and above.
The centre combines medical consultations with physiotherapy, nutrition support, supervised strength training and pain management. It also provides diagnostics, pharmacy services and rehabilitation before and after surgery. Care delivered at the clinic is connected to Praan Health’s digital platform, allowing prescribed programmes to continue at home.
The idea is to move away from a series of disconnected appointments. Instead, members receive a coordinated plan that can cover clinical treatment, mobility, nutrition and strength. Progress is assessed not only through health indicators, but also through practical changes such as reduced pain, improved movement and greater independence in everyday life.
Care designed around the person, not a single condition
Adults over 50 often seek treatment from different specialists for separate health concerns. Praan Health is positioning the new clinic as a single point of coordination, where multiple professionals work around a shared care plan.
Navneeth Ramprasad, founder and CEO of Praan Health, said the company’s experience showed that families needed a team willing to take responsibility for the broader outcome.
“India does not need another clinic that treats one symptom and leaves families to coordinate everything else,” Ramprasad said. He added that better ageing required attention to pain, mobility, strength, metabolic health and day-to-day independence.
The company said the Bengaluru centre was developed using insights from more than 350,000 personalised care sessions. It has previously supported over 10,000 adults aged 50 and above across more than 13 cities through its at-home model.
Under the expanded system, members can visit the clinic for consultations, assessments, diagnostics and specialised interventions. Physiotherapy, nutrition and strength programmes prescribed during those visits can then continue at home.
Helping families manage care from a distance
A notable part of the model is its focus on families who do not live in the same city or country.
In case of agreement from the individual, the family can still be linked up to the consultations and other related activities on the website. The plan has been devised such that the patient will have constant access to the professionals without necessarily having to coordinate all the appointments himself/herself.
This connection between the clinic, the home and the family is central to Praan Health’s approach. Rather than limiting treatment to a visit at the centre, the company plans to use the clinic as the clinical base for an ongoing programme of care.
Expansion planned across major cities
The Koramangala opening also marks Praan Health’s shift from an at-home chronic care startup towards a broader healthcare network combining physical clinics, technology and home-based services.
The company plans to introduce the model in Mumbai, Hyderabad and Delhi as part of a proposed expansion across major Indian cities by 2028.
Praan Health raised ₹8.5 crore in seed funding in November 2025. The round was led by Rainmatter Investments, with participation from WEH Ventures and angel investors. The company said the capital is being used to strengthen its clinical services, technology platform and physical care network.
For Praan Health, the Bengaluru clinic is both a physical expansion and a test of whether coordinated care can be delivered consistently across the clinic, the home and the wider family support system.
